>18 J., 2–18 J. und ≥40 kg (2–16 J.: nur Zusatztherapie): initial 2×tgl. 500 mg (alternativ: 2×tgl. 250 mg, nach 2 Wo. auf 2×tgl. 500 mg erhöhen), dann evtl. Tagesdosis alle 2–4 Wo. um 2×tgl. 250 mg oder 2×tgl. 500 mg erhöhen bis 2×tgl. 1500 mg.
6 Mon.–18 J. und <40 kg (6 Mon.–16 J.: nur Zusatztherapie): initial 2×tgl. 10 mg/kg, evtl. Tagesdosis alle 2–4 Wo. um 2×tgl. 10 mg/kg erhöhen bis max. 2×tgl. 30 mg/kg.
1–6 Mon. (nur Zusatztherapie): initial 2×tgl. 7 mg/kg, evtl. Tagesdosis alle 2 Wo. um 2×tgl. 7 mg/kg erhöhen bis max. 2×tgl. 21 mg/kg.
