6–18 ans et ≥18 à <35 kg: au début 40 mg 1×/j., entretien: max. 80 mg/j.
Insuffisance cardiaque
>18 ans: au début 40 mg 2×/j., puis augmenter par paliers jusqu'à max. 160 mg 2×/j.
Prévention après infarctus
>18 ans: début possible déjà 12 h après l'infarctus: au début 20 mg 2×/j., puis augmenter par paliers jusqu'à max. 160 mg 2×/j.
