1. Injektion: ca. 2 h vor Operation (allgemeine Chirurgie) bzw. 12 h vor Operation (Orthopädie, onkologische Chirurgie im Bauch- und Beckenbereich), Prophylaxedauer «FI».
Geringes Thromboembolierisiko: 1×tgl. 20 mg s.c.
Hohes Thromboembolierisiko: 1×tgl. 40 mg s.c.
Medizin
Prophylaxe venöser Thrombosen in der allgemeinen Medizin: 1×tgl. 40 mg s.c. während min. 6–max. 14 Tagen.
A. pectoris, nicht-Q-Wellen Myokardinfarkt: 1 mg/kg s.c. alle 12 h während 2–8 Tagen mit 1×tgl. 100–325 mg ASS p.o.
STEMI: 30 mg als Bolus i.v. + 1 mg/kg (max. 100 mg) s.c., dann 1 mg/kg (max. 100 mg) s.c. nach 12 h, dann 1 mg/kg s.c. alle 12 h, Therapiedauer max. 8 Tage, mit 1×tgl. 75–325 mg ASS sobald die Diagnose STEMI gestellt ist, >75 J.: 0,75 mg/kg s.c. alle 12 h (max. 75 mg für die ersten beiden Dosen) ohne initiale i.v. Bolus-Gabe. PCI «FI».
Venenthrombose: 2×tgl. 1 mg/kg s.c. (unkomplizierte Fälle evtl. 1×tgl. 1,5 mg/kg) während ca. 10 Tagen.
Hämodialyse: «FI».
