Monotherapie
>18 J.: initial 100 mg 1×tgl., Tagesdosis alle 2 Wo. um 100 mg erhöhen bis 300(–max. 500) mg.
Zusatztherapie (mit CYP3A4-Induktor)
>18 J.: initial 2×tgl. 25 mg, nach 1 Wo. auf 2×tgl. 50 mg erhöhen, ab 3. Wo. Tagesdosis wöch. um 100 mg erhöhen; Erhalt: 300–500 mg tgl. in 1–2 Gaben.
6–18 J. und >20 kg: initial 1×tgl. 1 mg/kg, Tagesdosis wöch. um 1 mg/kg erhöhen; Erhalt: >55 kg: 500 mg tgl., 20–55 kg: 8 mg/kg/Tag.
Zusatztherapie (ohne CYP3A4-Induktor)
>18 J.: initial 2×tgl. 25 mg, nach 2 Wo. auf 2×tgl. 50 mg erhöhen, ab 5. Wo. Tagesdosis alle 2 Wo. um 100 mg erhöhen; Erhalt: 300–500 mg tgl. (evtl. auch weniger) in 1–2 Gaben.
